Forms & Fees
All the forms needed to enroll your student will need to be printed, completed and mailed to the school. You will click on the hyperlink (blue highlighted/underlined) to open a printer friendly version of each form. If you do not have Adobe Acrobat Reader, you can download the free software.

New First Grade Students

High School Program
Our high school program is explained in the CLA High School Manual posted on the LINKS page of this website. You must read the manual and acquaint yourself with your responsibilities and the services CLA provides.
Please note: Enrollment will not be allowed if your student has had any disciplinary problems, expulsion, suspension, truancy, SARB review, etc. We will be verifying this information with student's previous school.
School Enrollment Fees
1st – 8th Grade: $85.00 per student/per school year
9th – 12th Grade: $225.00 per student/per school year
In order to provide a service that home school families can afford, CLA has kept enrollment fees as low as possible. Therefore we:
- Do not allow monthly installment payments of enrollment fees
- Do not pro-rate fees for families that enroll late in the year or refund fees to families that withdraw their student early in the school year.
Home School Legal Defense Association Membership
HSLDA membership is a mandatory requirement for enrollment in CLA. The membership application and fee must accompany your enrollment packet.
HSLDA Membership Application Form (new members only)
HSLDA Group Discount Enrollment Form (for current members under another discount group. You must transfer your membership to CLA’s group discount number. Print this form, fill it out and send it directly to HSLDA.)
Membership Fee: $95.00 Write a separate check to HSLDA and enclose it and application with your school enrollment packet.
