New Enrollment Forms
Proposed Course of Study (Print
one for each 1st – 8th grade student.)
This form is required by California law for each school to have on
file for each teacher. It is permissible to make changes in your curriculum
during the year. This is not “set in stone”! Course
of Study Required of Students in Private School (FYI)
High School Course Description/Course
Standard: (Template)
If you are enrolling a high school student you must submit course
description / course standards for each course your student will be
taking this school year. High School enrollment is not complete until
these forms are received. For information on “how to” do
cd/cs refer to the CLA
High School Manual.
Family Forms: (Print
one set per family)
Parental Responsibility Agreement: This form must accompany
your enrollment packet. Enrollment will not be completed without this
agreement/promise signed by both parents/or legal responsible parent/guardian
of the student.
Emergency Medical Form: California law requires that we have
this on file. This form is kept in the school’s file (not the
student’s) and is discarded/shredded once the student leaves
CLA. Fill it out completely.
Faculty Information Sheet: Required by law. Teachers in private
school are required “to be able to teach” (teaching degree
not required by law). This form is very self-explanatory giving you
a place to be very creative about you ability to teach: VBS, Sunday
School, team teaching situations. List any training: Home school workshops,
seminars, tapes, or books you’ve read.
Transfer of Records:
If your student attended another school, we will need to request their
records from that school. Please fill it out completely – especially
the previous school’s address.
If your student has never attended another school and you hold all
their records contact CLA for information on what you will need to
provide.
Print all the required form, fill them out and send
them to the school with enrollment fee.
